New Child Registration Form

To help make your visit as smooth as possible this Sunday consider filling out this form to register your children ahead of time. If completed, when you arrive you can proceed directly to our Children's Check-in Station in the front hall, enter the last four digits of your phone number and your children will be ready to join in the fun! We can't wait to meet you!

Purposes and Extent

Emmanuel Baptist Church is collecting and retaining this personal information for the purpose of enrolling your child in our programs, to assign the student to the appropriate classes, to develop and nurture ongoing relationships with you and your child, and to inform you of program updates and upcoming opportunities at our Church. This information will be maintained indefinitely as it is a requirement of our insurance company and legal counsel. If you wish Emmanuel Baptist Church to limit the information collected, or to view your child’s information, please contact us.

Waivers
I/we, the parents or guardians named, authorize the ministry staff of Emmanuel Baptist Church to sign a consent for medical treatment and to authorize any physician or hospital to provide medical assessment, treatment or procedures for the participant(s) named above.

As a result of the Global Pandemic, the government and local public health unit have made recommendations to prevent the spread of Covid-19. Signage is posted around the church, and we are doing everything we can to be compliant with all regulations and ensure your safety, but we cannot guarantee that you or your family members will not become infected with COVID-19.

I/we, undertake and agree to indemnify and hold harmless Ministry Personnel, Emmanuel Baptist Church, its Pastors and Board of Deacons from and against any loss, damage or injury suffered by the participant as a result of being part of the activities of the Emmanuel Baptist Church, as well as of any medical treatment authorized by the supervising individuals representing the Church. This consent and authorization is effective only when participating in or traveling to events sponsored by the Emmanuel Baptist Church.

I/we, the parents or guardians named, give Emmanuel Baptist Church permission to use photos, videos, and audio captured by our staff and volunteers that may include your child, for use by Emmanuel Baptist Church.

By registering your child and entering your name below you agree to the Purpose and Extent and Waivers as outlined above.
